T-1 Form – with Waiver and Assumption of Risk 2025
Managers be aware that if you choose to fill the Team Roster form out via computer, it will likely wrap to a second page to accommodate the needed space for the addresses and 20 players. If you choose to print the form and complete it by hand, the form won’t wrap but it will be difficult to get all of the required information in the spaces in a legible manner.
